This registered large medical waste generator does not maintain individual treatment records and shipping and <br /> tracking documents for 2 years. <br /> - A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records, and shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC 117975(a)—Corrective action submitted. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear-resistant. HSC <br /> 117630—Biohazardous waste bags used to collect waste that will be treated on site must meet ASTM standard <br /> D1709. Biohazardous waste that will be treated offsite must be contained in a bag that meets both ASTM <br /> D1709 and D1922. ASTM standards must be printed on the bag. Biohazardous waste bags used in the Dental <br /> Clinic of the"E" Yard did not have proper labeling. Implement use of correct bag in Dental clinic and ensure <br /> staff are equipped with the correct bags. Submit evidence of corrective action by 2/9/2018. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management <br /> plan. Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) <br /> -Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a <br /> container or secondary container labeled with the words "HIGH HEAT" or"INCINERATION ONLY"on the lid and <br /> sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B) - Corrected on site. <br /> 24. Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br /> - Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the <br /> words "Chemotherapy Waste" or"CHEMO" on the lid and sides, so as to be visible from any lateral direction. HSC <br /> 118275(a)(4)—Corrected on site. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and <br /> appropriately labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, <br /> and be kept clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or <br /> "BIOHAZARD" on the lid and on the sides so as to be visible from any lateral direction. HSC 118280(c)—Linens <br /> are being placed in a bag with a biohazard symbol on it and this bag is not placed in a rigid container. Halt <br /> use of biohazard bags for collection of linens, unless linens are medical waste at which point they must be <br /> treated and stored in a properly labeled bag placed in a properly labeled container. Submit evidence of <br /> corrective action by 2/9/2018. <br /> 26. Sharps waste is not placed in sharp container. Sharps container is not handled and/or labeled appropriately. <br /> - Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or <br /> tightly lid and stored onsite not more than 30 days. Sharps container shall be labeled with the words "Sharps Waste" <br /> or BIOHAZARD". HSC 118285—Corrected on site. <br /> Reinspecion on/about: A reinspection fee of$130 per hour may be charged.
