I am sending you this email to schedule a medical waste inspection for "California Health Care <br />Facility." Stephanie Peterson is listed as the facility contact. I left a voicemail for Stephanie on Monday <br />(1/13/2014). 1 called her again today, but she has an alternate message stating that she unavailable until January <br />28th. She listed you as an alternate contact. Can you provide me with dates on which we can schedule an <br />inspection? Due to the size of your facility it will most likely take more than one day to conduct the <br />inspection. We will be unavailable to conduct an inspection on January 28th due to an out of town meeting. <br />also had a question regarding your medical waste management plan. On page 6, question 3(f) and 3(g), of our <br />medical waste questionnaire, you are asked to list the offsite treament facility for biohazardous, sharps, and <br />pharmaceutical waste. In this section, this facility has listed "service contract in negotiation." This question asks <br />you to list the facility to which your hauler, "Converge Medical Solutions, LLC," hauls waste for treatment. Please <br />provide me with the name and site address of the offsite treament facility and update your plan to reflect this. <br />My office phone number is 209-468-3284. 1 am usually in the office in the mornings from 8am-10am and in the <br />afternoon from 330pm-430pm. I check my email and voicemail and the beginning and end of each day, so I am <br />usually able to respond to questions within 24 hours.
