In the locked common storage area, located in the dumpster area, the bagged <br />"Biohazard" waste will be placed directly into a leak -proof, rodent -proof, storage <br />container with a tight fitting lids identified and labeled with the words "Biohazard <br />Waste" and the international biohazard symbol. This container will be provided <br />and maintained by: <br />Stericycle, Inc -Autoclave <br />4. The storage containers will be located at the facility in the area of the dumpster <br />that is secured to deny access to unauthorized personnel. The area will be well <br />ventilated and kept clean at all times. The area will display a warning sign <br />declaring biohazard waste. <br />5. The storage containers will be picked -up intact (as a unit), as often as required, <br />but often enough to avoid the putrification of the waste contained therein. Pick up <br />will be one (1) time per week (every seven (7) days) by: <br />Stericycle, Inc -Autoclave <br />b. All used needles and syringes are to be considered medical waste. In accordance <br />with state law, they must be placed in a puncture proof "Sharps" container, which <br />prevents retrieval. When the "Sharps" container is 3/4 full, the container must be <br />sealed and placed in the "Medical Waste Storage" containers located in the utility <br />closets. <br />7. All pharmaceutical products being disposed of shall be stored in a locked closet in <br />the Director of Nurses office. The pharmaceuticals will have water poured over <br />them in the container to destroy the products. After destruction the discarded <br />pharmaceuticals shall be placed in a separate container from all other biohazard <br />waste and transferred to the outside locked biohazard storage area for pickup. <br />I hereby certify that to the best of my knowledge and belief, the statements made herein <br />are correct and true. <br />/67 <br />Administrator Signature Date <br />
