LOCKOUT TAGOUT POLICY AND PROCEDURES <br />4 <br />P. a <br />living centers <br />Attachment "C" <br />Outside Contractors' Policy and Procedures for Hazard Communication <br />And Lockout/Tagout <br />Policy <br />All outside contractors working in the facility will be briefed on the Hazard Communication and <br />lockout/tagout program and procedures that they will be required to follow. If the outside <br />contractors will be using their own Hazard Communication and lockout/tagout procedures, they <br />must brief the appropriate personnel. <br />Checklist <br />The checklist below must be completed each time services are to be performed by outside <br />contractors. Check off each stop as it is completed and attach to the Contractor <br />Acknowledgement Form. <br />Gotden Living I OCKOUTITAGOUT Page 9 of 10 <br />Procedure <br />1. Discuss the services to be performed by the outside contractor and the time frame <br />for the completion of the work. (This could be a one time job or monthly <br />procedure. <br />2. Use a floor plan of the facility to determine the locations in or around the facility <br />that will be affected by the work to be performed. <br />3. Determine the equipment and the employees that will be affected. <br />4. Review the Hazard Communication policy and how the contractor will be <br />affected. (Review the facility container labeling system, access to and location of <br />MSDS's, and any potential for chemicals in unlabeled pipes) <br />5. Review lockout/tagout policy and procedures. <br />6. Discuss and ensure that adequate worksite stands, signs or barricade tape are used <br />to secure the worksite as needed. Discuss any additional precautions necessary for <br />resident safety. <br />7. Have the contractor provide proof of insurance coverage according to the <br />guidelines established by the legal contracting department. <br />8. Make sure the general contractor understands that he/she is responsible for <br />insuring that these procedures are followed by his/her employees and/or any sub <br />contractors. <br />9. Have the contractor sign the acknowledgement form. Attach this completed <br />checklist to the signed acknowledgement and retain a copy in the Maintenance <br />Supervisor's office. <br />Gotden Living I OCKOUTITAGOUT Page 9 of 10 <br />
