The medical waste management plan (MWMP) does not contain all information listed in HSC117960. <br /> - MWMP shall contain all information listed in HSC 117960. HSC117960. The Medical Waste Management Plan estimates <br /> that only 1-1.5 pounds of medical waste is generated by the facility monthly. Submit an updated Medical Waste <br /> Management Plan which more accurately reflects the facility's generation (based on data from Stericycle)to the EHD by <br /> 2/27/19. <br /> 24. Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained <br /> pursuant to this section. The container shall be properly labeled. HSC 118275 <br /> The pharmaceutical waste container shall be labeled with the date on the date it is placed in use. Submit evidence of <br /> correction and in-service training to the EHD by 2/27/19. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tightfitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c) <br /> Two of the biohazardous waste containers in the outside storage are not labeled on the sides as to be visible from all <br /> lateral directions. Submit evidence of correction to the EHD by 2/27/19. <br /> 25. Biohazardous waste is stored above 32 F onsite for days. <br /> -A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br /> onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br /> store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) <br /> Since 7/17/17, the facility has generated over 20 pounds of biohazardous waste per month and held the waste onsite for <br /> up to 14 days above 32 F. Effective immediately, biohazardous waste is to be removed within 7 days of generation. <br /> Submit evidence of correction by 2/27/19. <br /> All corrections are due by 2/27/19. <br /> Notes: <br /> 1) Stericycle is the licensed hauler for biohazardous and pharmaceutical waste. The frequency of pick-ups is once every <br /> two weeks. <br /> 2) All sharps containers were changed prior to the inspection. <br /> 3) RCRA pharmaceutical waste is collected onsite in an appropriate container. <br /> 4) The biohazardous waste bags in use meet both the ASTM D1709 and ASTM D1922 standards. <br /> 5) Annual training records were available. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
