Do you handle pharmaceutical waste that is classified by the federal Drug Enforcement Agency(DEA) <br /> as "controlled substances"? <br /> Ifyes, describe how "controlled substances"are disposed. <br /> Hazardous Pharmaceutical Waste <br /> Phannaceutical wastes classified by the DEA as"controlled substance"shall be disposed of in <br /> compliance with DEA requirements and segregated for storage in accordance with the facility's <br /> medical waste management plan. <br /> Hazardous Pharmaceutical Waste is placed in an 8 or 18 gallon black Stericycle RC RA waste <br /> container that meet the EPA requirements for disposal of P,U and D-listed chemicals and DOT <br /> rated to transport hazardous waste. All are labeled"INCINERATION ONLY"so that it can be <br /> visible from any lateral direction. In addition, these single containers are still required to be labeled <br /> with the biohazardous waste symbol. The black RC RA waste container is stored in the Director of <br /> Nursing office lock cabinet and then transported to the designated accumulation area. Pharmaceutical <br /> waste that is separated from medical waste is maintained in a manner to secure the pharmaceutical <br /> waste contents from access by unauthorized individuals. Pharmaceutical waste containers that are full <br /> and ready for disposal may not be stored longer than 90 days. The container may not remain on site <br /> for more than a year whether full or not. When this to is prepared for shipment offsite for treatment, <br /> it is properly containerized for shipment in compliance with United States Department of Transportation <br /> and the Unites States Drug Enforcement Administration requirements.Hazardous pharmaceutical waste <br /> containers are disposed by calling the Stericycle's Specialty Waste division. Stericycle Specialty Waste <br /> Division will bring all shipping paperwork at the time of pickup. The transporter shall maintain a <br /> completed tracking document for the purpose of tracking the waste from the point when the waste leaves <br /> the generator facility until it receives final treatment. Appropriate trained employee at Vienna will sign <br /> the document and receive copies of the signed documents for the transportation and destruction of <br /> hazardous pharmaceutical waste. <br /> j. Describe training provided to staffregarding handling, storage, disposal, and record keeping of all <br /> medical waste, including pharmaceutical waste, at your facility: <br /> Training <br /> All employees are trained to recognize regulated medical and pharmaceutical waste and how to <br /> properly manage waste. Staff members who handle medical and pharmaceutical to are trained <br /> regarding personal protection and safety especially the proper use of personal protective equipment <br /> to wear, and proper handling, containerizing,disposing and labeling. The training focuses on staff <br /> members who(1)handle medical waste from the point-of-generation at the bedside,(2)transport the <br /> waste,and(3)prepare the waste for transport by contractor. In addition, the Director of Staff <br /> Development conducts in-services to all staff annually regarding Bloodborne Pathogens,Aerosol <br /> Transmissible Diseases,Handling Regulated Waste and any other measures taken to avoid exposure <br /> and emergency procedures in the event of exposure. A representative of Stericycle, Inc. also comes <br /> and conducts training yearly to all staff about Safety Data Sheets, Infectious Waste and Bloodborne <br /> Pathogen Safety. <br /> The Director of Staff Development keeps all training records of all staff in the Staff Development <br /> Office. <br />
