Section: <br />Infecti®n Control <br />General Policy Guidelines <br />Title: INFECTION CONTROL PLAN <br />Revision: <br />Reviewed/No <br />Changes <br />INFECTION CONTROL PLAN <br />The Infection Control plan shall Include: education, prevention, screening, surveillance, <br />investigation, tracking, trending, reporting and performance improvement. The plan is an <br />ongoing process designed to objectively and systematically monitor and evaluate the <br />effectiveness of the infection control plan and practice. The primary goal is the protection of <br />residents, fames / visitors and personnel from acquiring community and nosoGOmial <br />disease. The facility incorporates its infection Control plan as a major component of its <br />safety and performance improvement plan. <br />2. The facility performs an ongoing assessment to identify it's risks for the <br />acquisition and transmission of infectious .!- <br />-•- •I•r <br />ge <br />epemiological approach that consists of surveillance, data <br />collection and trend identificafio.T. <br />�. The facility effectively implements infection prevention and control processes <br />5. The facility collaborates with leaders across the organization to effectively participate <br />in the design and implementation of the infection control plan. <br />6.
