GROSS CONVALESCENT HOSPITAL
San Joaquin Local Health Dist.
1601 Hazelton Ave.
Stockton, CA 95201
M11IM111MIM
es-
I am sending you the enclosed application forms
for your approval. The procedure for disposal of
infectious waste for the Gross Convalescent Hospital
had been approved by your office in prior times. A
copy of your letter of approval of May 13,1977, is
attached.
It was not known to me that such requests must
be made annually. I am also enclosing for approval
written procedures for management of patients with
communicable diseases requiring isolation. This
facility does not admit patients with communicable
diseases so diagnosed prior to the admission.
Furthermore, it is our policy to transfer patients
to the acute hospital once an infection is acknow-
ledged by the attending physician. The treatment
under isolation procedures in this facility must
therefore be looked at as short term. During this
short period of time, however, this facility has
had a procedure by which patients were treated and
which I now do present to you for approval.
POST OFFICE BOX 67 • 321 WEST TURNER ROAD, LODI, CALIFORNIA 95240 0 (209) 369-3688
