This registered large quantity generator does not file a medical waste management plan (MWMP)with San Joaquin County <br /> Environmental Health Department(EHD). <br /> -A large quantity generator required to register with the EHD shall file with the EHD a MWMP on forms prescribed by the EHD. <br /> HSC117960. Annual training records for training covering the Medical Waste Management Plan were unavailable. <br /> Submit evidence of correction to the EHD by 2/20/19. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a). <br /> Shipping and tracking records were not available for the two weeks from 12/18/18 to 12/31/18. Submit evidence of pick- <br /> ups for the two weeks to the EHD by 2120119. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> -Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management plan. <br /> Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6). Pharmaceutical waste was discarded in <br /> three of the sharps containers. Submit evidence of an in-service on the segregation of pharmaceutical waste from <br /> sharps and biohazardous waste to the EHD by 2/20/19. <br /> All corrections are due by 2/20/19. <br /> Notes: <br /> 1) Barnett Medical Services is the licensed medical waste hauler for biohazardous and pharmaceutical waste. <br /> Biohazardous waste is picked-up once per week. Pharmaceutical waste is picked-up at least once per month. <br /> 2) The biohazardous waste bags meet both ASTM D1709 and D1922 standards. <br /> 3) The medical waste management plan is current and available on site. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
