HSC117960 — Review and revise MWMP as needed <br />and submit by 12/28/2015. <br />13. This registered large medical waste generator does not maintain individual treatment records and tracking <br />documents for 2 years. <br />- A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br />records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br />HSC117975(a) — Submit tracking documents or container detail report for 2014 to verify that waste was <br />removed once every 7 days. Obtain treatment documents from hauler. Submit evidence of compliance by <br />12/28/2015. <br />25. Biohazardous waste is stored above 32 F onsite for more than 7 days before removal. <br />- A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste <br />above 32 F onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month <br />shall not contain or store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) —Facility was <br />unable to provide records for 2014 to verify that waste was removed at least once every 7 days. Submit <br />tracking documents or a container detail report to verify that waste was removed once every 7 days by <br />12/28/15. <br />Notes: <br />1.)
