II 10010 1 <br />Policy <br />Discontinued medications not sent with the resident on discharge, and medications left in the facility after a <br />resident's discharge which do not qualify for return to the pharmacy for credit, are destroyed via Cal -OSHA - <br />compliant procedures and via a licensed medical waste hauler. <br />Procedures <br />A. All medications shall be released to the resident upon discharge unless they have been <br />discontinued, the directions for use are significantly different from the label on the container, the <br />resident leaves against medical advice, or the prescriber states otherwise. <br />B. Release of medications to the resident or resident's responsible party shall be documented on the <br />MEDICATION DISPOSITION FORM and include the quantity released, medication, strength, <br />dose, and directions for use. <br />C. All non -controlled drugs that are eligible for disposal are placed in an approved waste container <br />properly labeled as . The provider pharmacy is <br />contacted if the facility is unsure of proper disposal methods for a medication.* <br />D. Medication destruction occurs only in the presence of two licensed nurses or one licensed nurse <br />and a pharmacist and is documented on the MEDICATION DISPOSAL FORM. <br />E. Controlled substances are retained in a securely locked area with restricted access. (see IE1: <br />CONTROLLED MEDICATION DISPOSAL).* <br />F. All controlled drugs are placed in an approved waste container properly labeled as medication <br />waste or pharmaceutical waste. The container shall be sealed and scheduled for destruction via <br />the facility's approved waste hauler. <br />G. The RN and pharmacist witnessing the destruction ensures that the following information is <br />entered on the controlled drug record or on a medication disposition form (see Form 11: <br />CONTROLLED DRUG RECORD): <br />1) Date of destruction <br />2) Resident's name <br />3) Name and strength of medication <br />4) Prescription number <br />5) Amount of medication destroyed <br />6) Signatures of witnesses <br />PHARMACY ADVANTAGE <br />73 <br />Effective Date: April 1, 2008 <br />
