A Sharps Injury Log must be maintained for the recording of percutaneous injuries <br /> from contaminated sharps. The log shall contain, at a minimum, the type and brand of <br /> device involved in the incident, work area where the exposure incident occurred and an <br /> explanation of how the incident occurred. The information contained on the log must <br /> be maintained in a manner that protects the privacy of the injured employee. The <br /> Infection Prevention Committee will review the logs to evaluate the risk and device <br /> effectiveness. <br /> 16. The Infection Prevention Committee uses the investigation of the exposure incident <br /> and the Sharps Injury Log review to identify/correct problems to prevent similar <br /> incidents from recurring. <br /> 4.0 DOCUMENTATION <br /> 1. An exposure incident is reported on Part A, Occupational Exposure Incident Report. <br /> 2. Evaluation of the incident is recorded on Part B, Post-Exposure Incident Evaluation. <br /> 3. Obtain source's permission for testing on Source Individual's Consent or <br /> Declination to HIV/HBV Testing (Briggs). <br /> 4. Information pertinent to the employee's medical evaluation is recorded on Information <br /> to Health Care Provider (Briggs). <br /> 5. The results of the post-exposure medical evaluation are provided to the employee on <br /> Health Care Professional's Written Opinion (Briggs). <br /> 6. Complete the Sharps Injury Log and forward to the Safety Committee.
