This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> - A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a) <br /> — Facility only had treatment records for pharmaceutical waste. Print treatment records for all medical waste <br /> removed by hauler and maintain on site for 2 years. <br /> —Facility was able to show that they have access to all treatment records on line and are able to print them <br /> from this point forward. <br /> Notes: <br /> 1.) Facility contracts with Stericycle for the removal of medical waste. Currently, medical waste is picked up twice a <br /> week by Stericycle. <br /> 2.)
