This registered large medical waste generator does not maintain individual treatment records and <br /> shipping and tracking documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a). <br /> Shipping and tracking records are not available for the weeks of 6/4/18, 9/24/18, and 11/9/18. Submit evidence of pick- <br /> up by a licensed medical waste by 2/19/19. (This violation was corrected during the inspection). <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tearresistant. <br /> HSC 117630. The small biohazardous waste bags meet the ASTM D1709 standard, and can be used for collection of <br /> biohazardous waste within the facility. The small bags must be placed in a bag that meets both the ASTM D1709 and <br /> D1922 standards prior to shipping the waste off site. The large bags do not meet ASTM standards D1709 or D1922,and <br /> should not be used in the facility. Replace the large biohazardous waste bags with approved bags. Submit evidence of <br /> correction to the EHD by 2/19/19. <br /> 24. Consolidated waste is not treated by incineration. The container of consolidated waste is not <br /> labeled appropriately. <br /> -Consolidated waste shall be treated by incineration, and the container shall be labeled with the biohazardous waste symbol and <br /> the words"HIGH HEAT OR INCINERATION ONLY"on the lid and side so as to be visible from any lateral direction. HSC <br /> 118275(a)(7). The pharmaceutical waste containers in use are not labeled "High Heat"or"Incinerate Only"on the lid <br /> and all sides. The expired medications are collected in an unapproved container. Containerize the expired <br /> medications in an approved container with proper labeling. Submit evidence of correction to the EHD by 2/19/19. <br /> All corrections are due by 2/19/19. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler for biohazardous and pharmaceutical waste. The facility has waste <br /> picked-up once per week. <br /> 2) Pharmaceutical waste was picked-up once per month with the exception of June 2018 and October 2018. <br /> 3) All pharmaceutical waste is disposed of as RCRA regulated waste. <br /> 4) Annual training records are available for the 7 employees which handle medical waste. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
