WASTE DISPOSAL AGREEMENT <br />between <br />CRESTWOOD MANOR CONVALESCENT HOSPITAL <br />1130 Monaco Court <br />Stockton, California <br />and <br />CRESTWOOD CONVALESCENT HOSPITAL <br />442 E. Hampton <br />Stockton, California <br />As Crestwood Manor Convalescent Hospital has an incinerator <br />approved by the San Joaquin County Air Pollution Control District <br />the incineration of Crestwood Convalescent Hospitals Infectious <br />Materials (Dressings) may take place under the following conditions: <br />1. The materials shall be packaged or wrapped securely in heavy <br />wrapping paper, utilizing a sufficient number of thicknesses <br />to prevent the seepage of :any moisture, liquid or blood from <br />penetrating the outer wrapping. Newspaper may be used for the <br />inner wrapping. <br />2. The paper wrapped packages shall be placed in plastic bags <br />and sealed. <br />3. The paper wrapped packages shall be adequately sealed with <br />adhesive tape to prevent rupture or opening of the packages in <br />ordinary handling. <br />4. The bags shall be marked "Contaminated Material - Do Not Open." <br />5. Such packages shall be stored in a tightly covered metal garbage <br />container and removed at ,least twice a week, <br />A <br />
