This registered large medical waste generator does not maintain individual treatment records and tracking <br />documents for 2 years. <br />- A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br />records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br />HSC 117975(a) — Facility was not maintaining treatment records on site. During the inspection, facility was <br />able to obtain treatment records from Stericycle. Ensure records are maintained on site for 2 years. Violation <br />was corrected on site. <br />Notes: <br />1.) Facility contracts with Stericycle for the removal of medical waste. <br />2.) Biohazardous waste bags on site meet both ASTM standards (D 1922, D 1709) <br />3.) Medical Waste Management Plan was on site and available for review. <br />4.) Facility has 2 designated accumulation areas located indoors. Majority of the biohazardous waste generated at <br />this facility is sharps waste. Pharmaceutical waste containers stored in locked drawers of medication carts and <br />in locked rooms at nurse's stations. <br />Reinspecion on/about: A reinspection The of $130 per hour may be charged.
