The medical waste management plan (MWMP) does not contain all information listed in HSC 117960. <br />- MWMP shall contain all information listed in HSC 117960. HSC117960 — Submit revised MWMP by 12/23/2015. <br />13. This registered large medical waste generator does not maintain individual treatment records and tracking <br />documents for 2 years. <br />- A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br />records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br />HSC117975(a) — Obtain records showing treatment of waste from hauler. Submit evidence of compliance by <br />12/23/2015. <br />23. Biohazard bag does not meet the standard set forth in HSC 117630. <br />- Biohazard bags containing medical waste which will be sent offsite for treatment shall be marked and cetified by the <br />manufacturer as having passed ASTM D1922 and ASTM D1709. When collecting medical waste within the facility, <br />the waste may be placed in a biohazard bag which meets ASTM D 1709 provided that when the bag is prepared for <br />transport offsite, it is placed into a USDOT-approved container line with a biohazard bag that is ASTM D1709 and <br />ASTM D 1922 certified. HSC 117630 — Submit evidence of compliance by 12/23/2015. <br />24. Pharmaceutical waste container located in the procedure room was not labeled properly. <br />- Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a <br />container or secondary container labeled with the words "HIGH HEAT" OR "INCINERATION ONLY" on the lid and <br />sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B) — Submit evidence of compliance by <br />12/23/2015. <br />25. Biohazardous waste is stored above 32 F onsite for 2 weeks before removal. <br />- A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste <br />above 32 F onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month <br />shall not contain or store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) — Facility was <br />unable to provide records showing that medical waste was removed every 7 days on several occasions between <br />June 2014 and March 2015. From March 2015 forward, facility has had pickups conducted every 7 days and <br />all records were available. No corrective action needed. <br />Notes: <br />1.)
