I have also attached a Medical Waste Management Plan (MWMP) <br /> handout. <br /> The main issue during the inspection was that the tracking and treatment records from the medical waste hauler were <br /> not available for review. When medical waste is picked up from the facility,a tracking document is left behind. Once <br /> the waste is treated,that tracking document is signed by the treatment facility verifying destruction and thus becomes a <br /> "treatment" document. Companies have different methods (mail, email,fax, online etc.) of routing the treatment <br /> record back to the customer. I'm not sure which method your hauler would use. The facility must maintain 2 years of <br /> tracking and treatment records on site. To correct the violation for not having the tracking and treatment records and <br /> the violation for 7 day removal,you will need to contact the hauler, obtain the treatment records, and submit them as <br /> evidence of compliance. <br /> The second issue during the inspection was that the Medical Waste Management Plan was unavailable for review. We <br /> have template that I have attached to this email. If all of the questions from pages 5-8 are filled out and submitted,this <br /> meets all of the minimum requirements for a Medical Waste Management Plan. <br /> If you have any questions please let me know. <br /> Thank you, <br /> Harprit S.
