The medical waste management plan (MWMP) does not contain all information listed in HSC 117960. <br /> - MWMP shall contain all information listed in HSC 117960. HSC 117960—MWMP was unavailable for review. <br /> Submit MWMP by 10/14/2016. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)— Tracking records for 2015 were unavailable for review. Several tracking documents for 2016 <br /> were unavailable. No treatment records were available for review. Obtain tracking records showing treatment <br /> of waste. Biohazardous waste is stored above 32 F onsite for more than 7 days before removal. <br /> - A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste <br /> above 32 F onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month <br /> shall not contain or store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1)—Submit <br /> documentation showing that medical waste is removed once every 7 days. Sumbit evidence of compliance by <br /> 10/14/2016. <br /> Notes: <br /> 1.) Facility contracts with Daniels Sharpsmart for the removal of medical waste. <br /> 2.) Facility generates biohazardous waste, biohazardous sharps waste, and pharmaceutical waste. <br /> 3.) Biohazardous waste bags meet ASTM standards D1709 and D1922. <br /> 4.) A full pharmaceutical waste container was observed on site. Pharmaceutical waste containers must be picked <br /> up within 90 days once full. Any pharmaceutical waste container containing waste must be picked up at least <br /> once every year regardless of the amount of waste inside. <br /> Reinspecion on/about: A reinspection fee of$130 per hour may be charged.
