Prima has just <br /> recently begun to service the central valley, so the driver is getting used to the locations and tracking <br /> documents. The waste is taken to NGG's Chowchilla transfer station. The driver George, did grab the <br /> wrong stack of papers, and gave them to New Hope. I have attached New Hope's actual tracking from <br /> Prima. From reviewing the customer reports, I am confident that Prima is not subcontracting. It is <br /> good that you were aware of those that do actually subcontract. Also, the statute section for 118280 <br /> will probably change this year, so I wouldn't keep adding the "sharps" when you give the violation, since <br /> it adds confusion. The error will probably be fixed. Sharps always have 30 days. I do understand that <br /> you might have forgot as you were writing it out. <br /> NGG mentioned also that New Hope will probably need, as you have reflected, a weekly pickup, since <br /> they have enough beds to push them over the weight limit. I performed an inspection on April 10, 2014 at New Hope Care Center(see attachment), a skilled <br /> nursing facility, in Tracy California and found discrepancies. New Hope has a service agreement(see <br /> attachment)with Prima Medical Waste Management for bi-weekly pick-up to start March 10, 2014. Per the facility <br /> only one tracking document was left onsite for the 3 pick-ups of medical waste. The tracking document shows a <br /> pick up by National Green Gas was performed on March 24, 2014 (see attachment). The facility contacted <br /> National Green Gas to request tracking documents for 2 other pick-ups performed on March 10, 2014 and April 7, <br /> 2014. <br /> 1 noted on my report that a phone call made, during the inspection, by New Hope to Prima Medical Waste <br /> Management to ask why National Green Gas tracking documents are being used, A Mr. Fernando stated the <br /> driver provided an incorrect one. I also noted on my report that a phone call made, during the inspection, by <br /> National Green Gas to New Hope, informed Al Matthews, New Hope representative, that the driver made a <br /> mistake and provided the wrong tracking document. <br /> Based on information from those phone calls, it appears a driver is carrying Prima Waste Management tracking <br /> documents and National Green Gas tracking documents, is this allowed? Can New Hope have National Green <br /> Gas (a hazardous waste hauler) listed in their Medical Waste Management Plan as their hauler, have a service <br /> agreement with National Green Gas and have tracking documents from National Green Gas for medical waste <br /> removed but the vehicle used to remove the waste belongs to Prima Medical Waste (a hazardous waste hauler)? <br /> Alfonso Arambula, Sr RENS <br /> 4/14/2014 <br />
