0 <br />CPR Pharmaceutical Reverse Distribution Service Agreement Page 5 of 7 <br />Guaranteed <br />Returnso <br />Detailed Description <br />• All CII -CV's, RX, OTC & HBC Processed for credit. <br />• All creditable and non -creditable items invoiced back to each manufacturer. <br />• All DEA 222 Forms & DEA 41 forms completed and issued to each pharmacy. <br />• ARCOS reporting completed. <br />• Unique Bar -Coded Return Forms & Shipping labels utilized by each pharmacy per carton. <br />• Forms & Labels shipped to each pharmacy prior to each service cycle. Form & Label Binders available <br />upon request. Each binder includes CII Inventory section, CIII-CV inventory section, recall section, <br />shipping labels, re -order forms, and instructions for each section. <br />• All cartons electronically received. Each carton receives unique "box -in number". <br />• All items are linked to each unique box -in number. <br />• Each pharmacy can elect to have an on-site Guaranteed Returns' Representative to service their <br />pharmacy <br />• The Representative will review program benefits and answer any questions. <br />• The Representative will sweep pharmacy shelves, inventory controls and execute a DEA 222 Form. (All <br />Guaranteed Returns® Representatives are fully trained, licensed and have Durable Power of Attorney). <br />• <br />All cartons will be properly packed and labeled. The Representative will contact UPS or FedEx for pick <br />up at pharmacy. <br />• For an additional fee, the Representative can take a full inventory of all items being shipped to <br />Guaranteed Returns'. <br />
