BFI Medical Waste Systems of CA., Inc. <br />7] <br />S-A T E OF CALiFOR(`l1A <br />Department of Corrections <br />MEDICAL WASTE REMOVAL AND DISPOSAL <br />Contract Number MLT97000 <br />Attachment <br />The undersigned hereby proposes and agrees to furnish all labor, materials and <br />equipment necessary to perform all services required for the foregoing titled work in <br />accordance with the Instructions to Bidders, Scope of Services, <br />GenerTerms and <br />Conditions, Minority, Women and Disabled Veteran Business Enterp sea(M/N//DVBE) <br />requirements, if applicable, and such addenda thereto as may be issued prior to the <br />public bid opening date for the rates set forth in the spaces provided herein. <br />The rates set forth shall remain in force for the stated term of the can and shall <br />include the cost of insurance and every other item of expense, direct or indirect, including <br />State sales tax incidental to the bid price. <br />Rates MUST be provided for all services listed below. Failure to provide the required <br />rates shall be cause for rejection of your bid. <br />SUPPLIES/EQUIPMENT/FIXED TtNSPORTATlnN AND DISPOSAL t:H�iRGES <br />' ► he fixed charge per pound shall include the costs of labor, containers, plastic bacs <br />labels, storage bins and lockers (existing and non-existin ' <br />gand <br />other supplies, equipment; and shall include the cost to transpop medical wase frolm the <br />institution facilities to a permitted medical waste TSO facility, the cost of incineration of <br />the medical waste, the cost of the final disposition of the medical waste and the medical <br />waste ash residue, and all time and labor charges for repacking and relabeling (if <br />necessary), loading, and handling of medical waste, containers; and preparation of all <br />Paperwork associated with such activities. <br />BASlS OF AWARD <br />The Department of Corrections intends to award one or more contract(s) to the lowest <br />responsible bidder(s). The State reser®es the right to award either 1) one ccritract for all <br />institutions, 2) one contract per geographic region 3) one contract per institution. This <br />determination will be based on what is in the best interest of the State. <br />9idders may submit bids for any or all institutions. If a bidder is the lowest bidder for <br />more than one institution, all of the institutions awarded to that bidder wiil be incorporated <br />into one contract. <br />Bidders .may provide a percentage discount for being awarded multipleinstitutions. !f <br />diccers chose to give a discount, the lowest respons;ble bidcer gov,ng the highest <br />liscount may be awarced the contract. <br />
