This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a)— <br /> Tracking record for 10/14/21 was incomplete. Contact medical waste hauler for treatment record and provide a copy to <br /> the EHD <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or"BIOHAZARD" on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c)—The labels on the Biohazardous waste containers next <br /> to bed#1 and bed#2 is worn. The lid of the secondary container in the accumulation/stc rage room was open. Replace <br /> labels on the top of the biohazardous waste containers and ensure the lids of secondary containers are closed. Provide <br /> photographic evidence of correction to the EHD. <br /> 25. Biohazardous waste is stored above 32 F onsite for days. <br /> -A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br /> onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br /> store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1)—Tracking records indicated there were no <br /> medical waste pickup between the following dates,which is over 7 days: <br /> September 23, 2021 to October 7, 2021 <br /> November 22, 2021 to December 2, 2021 <br /> December 9, 2021 to December 30, 2021 <br /> Ensure medical waste is picked up every 7 days. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by January 28, 2023. <br /> Notes: <br /> 1. Please note, per section 117970 of the Medical Waste Management Act: Generators shall update their medical waste <br /> management plan when any of the information in the plan changes and shall have the plan on file for review during an inspection <br /> or upon request. The updated plan shall be submitted to the EHD within 30 days of the change. <br /> 4.
