HSC117960—Submit revised MWMP by 12/17/2015. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)—During this inspection,facility called Stericycle and obtained treatment records. No <br /> corrective action needed. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant,tightly lid, clean, in good repair and <br /> appropriately labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant,have tight-fitting cover, <br /> and be kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or <br /> "BIOHAZARD"on the lid and on the sides so as to be visible from any lateral direction. HSC 118280(c)—Facility <br /> lableled containers during the inspection. No corrective action needed. <br /> 28. Reusable pails, drums, dumpsters, or bins used for medical waste are used for solid waste containment without <br /> being properly decontaminated. <br /> -A person shall not use reusable pails, drums,dumpsters, or bins used for medical waste for the containment of solid <br /> waste, or for other purposes, except after being decontaminated by the procedures specified in HSC 118295 and <br /> removal of all medical waste labels. HSC 118305—Remove all biohazard labels from medical waste containers <br /> prior to using them for solid waste containment. If the container will be used to store medical waste,it must be <br /> properly labeled.
