This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)—Treatment records were not available for review. Submit treatment records by 10/12/16. <br /> Notes: <br /> 1.) Biohazardous waste bags must meet ASTM standards D 1709 and D 1922 for waste being transported off site. <br /> The large bags on site meet these standards. Biohazardous bags used solely to store waste on site only need to <br /> meet ASTM D1709. When purchasing new small biohazardous waste bags,the bags can meet ASTM D1709 <br /> as long as the waste is placed into the larger bags which meet both standards prior to pickup by Stericycle. <br /> 2.)
