This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite <br /> for treatment for 2 years. HSC117975(a) No shipping and tracking records are available for the period of 11/28/18 to <br /> 12124/18. Submit evidence of shipping and tracking records for the period of 11/28/18 to 12/24/18 to the EHD by 2/7/19. <br /> 24. Pharmaceutical and medical waste shall be segregated and contained pursuant to this section. The container shall be <br /> properly labeled HSC 118275 The pharmaceutical waste container was not labeled with the date that it was put in use. <br /> Submit evidence of correction to the EHD by 2/7119. <br /> All corrections are due by 2/7/19. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler. Medical waste is picked-up twice per week. <br /> 2) Pharmaceutical waste is picked-up at least once every 90 days. <br /> 3) The Medical Waste Management Plan is available onsite, and annual training records are available for the 8 employees <br /> which come in contact with medical waste. <br /> 4) The biohazardous waste bags in use meet both the ASTM D1709 and D1922 standards. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
