The medical waste management plan (MWMP)does not contain all information listed in HSC 117960. <br /> -MWMP shall contain all information listed in HSC 117960. HSC117960—Submit MWMP by 12/18/2015. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)—Facility was able to log onto hauler's website and obtain treatment records. No further <br /> corrective action needed. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Biohazard bags containing medical waste which will be sent offsite for treatment shall be marked and cetified by the <br /> manufacturer as having passed ASTM D1922 and ASTM D1709. When collecting medical waste within the facility, <br /> the waste may be placed in a biohazard bag which meets ASTM D1709 provided that when the bag is prepared for <br /> transport offsite, it is placed into a USDOT-approved container line with a biohazard bag that is ASTM D1709 and <br /> ASTM D1922 certified. HSC 117630—Submit evidence of compliance by 12/18/2015. <br /> 24. Pharmaceutical waste container located in the procedure room was not labeled properly. <br /> -Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a <br /> container or secondary container labeled with the words"HIGH HEAT"OR"INCINERATION ONLY"on the lid <br /> and sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B)—Submit evidence of compliance by <br /> 12/18/2015. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant,tightly lid, clean, in good repair and <br /> appropriately labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, <br /> and be kept clean and in good repair. Container shall be labeled with the words`Biohazardous Waste" or <br /> "BIOHAZARD"on the lid and on the sides so as to be visible from any lateral direction. HSC 118280(c)—Label <br /> containers and submit evidence of compliance by 12/18/2015. <br /> 26. Sharps waste is not placed in sharp container. Sharps container is not handled and/or labeled appropriately. <br /> - Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or <br /> tightly lid and stored onsite not more than 30 days. Sharps container shall be labeled with the words"Sharps Waste" <br /> or BIOHAZARD". HSC 118285—Either ensure containers are placed so that labels are visible,or label all sides <br /> of the containers. Submit evidence of compliance by 12/18/2015. <br /> Notes: <br /> 1.)Facility contracts with Stericycle for the removal of medical waste.
