This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a)— <br /> Tracking records were provided from January 2023 to March 2023. Stericycle was contacted for the treatment records, <br /> during the inspection. Provide EHD with tracking and treatment records from January 2022 to present. <br /> 24. Medical waste is not segregated at point of generation. <br /> - Medical waste shall be contained separately from other waste at the point of origin. HSC 118275(a)(1)—The sharps <br /> containers near stations 12 and 11 consisted of bloody gauze pads. In-service staff on proper medical waste <br /> segregation and provide training record(s)to the EHD. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BI DHAZARD" on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c)—The biohazard container located near station 12 was <br /> not labeled properly.The label on the front of the container was missing. Replace biohazard label on the container and <br /> provide photographic evidence of correction to the EHD. <br /> 31. The biohazard bag that is used to collect medical waste within a facility shall be manufacturer certified to meet the ASTM <br /> D1709 dart drop test, provided that when the bag is prepared for transport offsite, it is placed into a USDOT—approved <br /> container lined with a biohazard bag that is ASTM D1709 and ASTM D1922 certified. HSC 117630—The secondary container <br /> was not lined with a biohazard bag and the top of the container was not closed. Per Facility Administrator, biohazard <br /> bags have been ordered for the secondary containers, but they have not been delivered to the facility yet. Ensure the <br /> secondary container is lined with a biohazard bag and the top is closed when the container is used. Provide EHD with <br /> photographic evidence of corrections. <br /> 28. Reusable pails, drums, dumpsters, or bins used for medical waste are used for solid waste containment without being <br /> properly decontaminated. <br /> -A person shall not use reusable pails, drums, dumpsters, or bins used for medical waste for the containment of solid waste, or <br /> for other purposes, except after being decontaminated by the procedures specified in HSC 118295 and removal of all medical <br /> waste labels. HSC 118305—A biohazard labeled container located outside of the breakroom was being used for face <br /> mask disposal.Solid waste should not be disposed in a biohazard container. Remove all labels from the container and <br /> provide photographic evidence to the EHD. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by March 25, 2023. <br /> Reinspecion on/about: A reinspection fee of$156 per hour may be charged.
