The medical waste management plan (MWMP) does not contain all information listed in HSC 117960. <br />- MWMP shall contain all information listed in HSC 117960. HSC117960 <br />The Medical Waste Management Plan requires updating with information on AGR which is used to ship biohazardous <br />waste off site in addition to Stericycle which is the primary medical waste hauler. Submit an updated copy of the <br />Medical Waste Management Plan to the EHD by 3/8/19. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) <br />The ice chest in the specimen room is not labeled with biohazardous waste signs as to be visible from all lateral sides. <br />Submit evidence of correction to the EHD by 3/8/19. <br />Note: <br />1) Stericycle is the primary medical waste hauler for the facility. Medical waste is picked -up from the facility once per week. <br />The biohazardous waste picked -up by Stericycle consists of waste generated during practice surgeries and surgical <br />specimens. The surgical specimens picked -up by Stericycle without respect to quantity or weight are (1) hands, wrists <br />and elbows, (2) shoulders, (3) hips, (4) feet and ankles, and (5) knees. All surgical specimens are shipped to Oregon <br />and incinerated. The other waste is shipped to Stericycle facilities within California. <br />2) AGR is a secondary medical waste hauler for the facility. AGR is both the source and the medical waste hauler for the <br />torsos used as surgical specimens in the facility. The torsos are returned to AGR as waste for incineration. Some <br />specimens are used more than once. Once they are deemed waste, they are picked -up by AGR. <br />3) The facility uses hampers with red bags for containerizing biohazardous contaminated towels which are then washed <br />and reused. The hampers should use yellow biohazardous waste bags for contaminated towels, and only use red <br />biohazardous waste bags for shipping waste off site. <br />4) Medical waste is stored in the surgical lab, the specimen room, the men's and women's restrooms, and the laundry <br />room. The biohazardous waste containers in the restrooms are used for disposable scrubs and sharps. The <br />biohazardous waste container in the laundry room is for sharps. <br />5) The red biohazardous waste bags in use in the facility for shipping waste off site meet both the ASTM D1709 and ASTM <br />D1922 standards. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
