For steam sterilization, a standard written operating procedure for biological indicator is not available. <br /> -Standard written operating procedures shall be established for biological indicators, or for other indicators of adequate <br /> sterilization for each steam sterilizer, including time, temperature, pressure, type of waste, type of container, closure on <br /> container, pattern of loading, water contents, and maximum load quantity. HSC 118215(a)(2)(A) <br /> Provide a written copy of the operating procedure for the autoclave along with evidence of annual training for the <br /> operator. Submit to the EHD by 2/25/2020. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD"on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c) <br /> 1. Biohazard waste from the Physiology Classroom was stored in a red bag on the floor in the First Floor Prep Room. <br /> Provide an appropriately labeled rigid container with liner for the collection of biohazard waste. Submit photographic <br /> evidence of correction to the EHD by 2/25/2020. <br /> 2. Biohazard waste in the Microbiology Classroom (#210)was stored in a red bag on the floor. Provide an appropriately <br /> labeled rigid container with liner for the collection of biohazard waste (This violation was corrected during the inspection). <br /> 3.Two different biohazard waste bags are in use in the facility. The bags purchased by the college meet both the ASTM <br /> D-1709 and ASTM D-1922 standards. Ingenium supplies unmarked bags made by VWR(14220-084). Submit evidence of <br /> compliance for the VWR 14220-084 bags to the EHD by 2/25/2020. <br /> All corrections are due by 2/25/2020. <br /> Photographic evidence of correction can be submitted to the EHD by email at: =aster(Dsigov.orq <br /> Notes: <br /> 1) Ingenium is the licensed medical waste hauler for this facility. Medical waste is collected once per week. <br /> 2) Shipping and tracking records were available for review. <br /> 3) The Medical Waste Management Plan was available,and the copy in the department file is current. <br /> 4) The Sterilization log was available for review along with monthly spore testing results and evidence of <br /> calibration of the autoclave thermometers in the last year. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
