Biohazardous waste is stored above 32 F onsite for days. <br /> -A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br /> onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br /> store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) <br /> Biohazardous waste was not picked up from the facility in January of 2019. Biohazardous waste may not be held on <br /> site for over 30 days. Submit evidence of a change in the frequency of pick-ups by Stericycle to once per month to the <br /> EHD by 2/28/19. <br /> All corrections are due by 2/28/19. <br /> Notes: <br /> 1) Per request, the facility requested a weight report from Stericycle. The report shows that in the last two years, the facility <br /> has not generated over 24.3 pounds of biohazardous waste, and that the facility only produced over 20 pounds of waste <br /> in 5 out of the last 29 months. The facility permit should be amended to reclassify the facility as a small quantity <br /> generator without treatment. A completed copy of the Medical Waste Generator Certification Statement was obtained <br /> during the inspection. <br /> 2) Stericycle is the license medical waste hauler for biohazardous and pharmaceutical waste. The facility had <br /> biohazardous waste picked-up once per week from January 2017 through April 2018. The frequency of pick-ups was <br /> changed to once per week in May 2018, and then changed to once every two months in January 2019. No pick-up <br /> occurred in January 2019. <br /> 3) All pharmaceutical waste is disposed of as RCRA pharmaceutical waste. <br /> 4) The biohazardous waste bags in use in the facility meet both the ASTM D1709 and D1922 standards. <br /> 5) Annual training records were available for review. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
