ENDOSCOPY CENTER OF LODI <br /> Title: Hazardous Materials and Regulated POLICY: EOC 300 <br /> Medical Waste Management Plan Page 1 of 6 <br /> State: EFFECTIVE: 4/19/2012 <br /> CMS: 416.42, 416.44 a 3 , 416.49(b)(2) 416.51 REVISED: <br /> TJC: EC.02.01.01.01 <br /> AAAHC: 7-I,J 8-M <br /> OPURPOSE: <br /> This hazardous materials and waste management plan describes how the center will <br /> establish and maintain a program to safely control hazardous materials and waste. <br /> This plan will comply with all Regulatory Agencies; City, County, State, Federal, NIOSH <br /> (National Institute for Occupational Safety and Health), OSHA (Occupational Safety and <br /> Health Administration), and NFPA (National Fire Protection Association) Standards. <br /> POLICY: <br /> This hazardous materials and waste management plan is facility-wide in scope. This <br /> plan is for the Endoscopy Center of Lodi located at 840 S. Fairmont Ave, Suite 1, Lodi, <br /> CA 95240; an ambulatory surgery center. <br /> The responsible person for implementation of this plan is the Administrator of the <br /> facility. <br /> Medical waste tracking documents will be maintained for three years at the facility. <br /> PROCEDURE: <br /> 1. The center Hazardous Materials and Regulated Medical Waste Management <br /> planning includes identifying processes for: <br /> a. Selecting, handling, storing, transporting, using, and disposing of <br /> hazardous materials and waste from receipt or generation through use or <br /> final disposal; <br /> b. Establishing written criteria consistent with applicable law and regulation <br /> (for example, Environment Protection Agency, Occupational Safety and <br /> Health Administration) to identify, evaluate, and inventory hazardous <br /> materials and waste used or generated; <br /> c. Managing chemical waste, and Regulated Medical Waste, including <br /> sharps; <br /> d. Providing adequate and appropriate space and equipment for safe <br /> handling and storage of hazardous materials and waste; <br /> e. Reporting and investigating all hazardous materials or waste spills, <br /> exposures, and other incidents; <br /> f. Monitoring and disposing of hazardous gases and vapors; and <br /> g. An annual evaluation of the hazardous materials and waste management <br /> program's objectives, scope, performance, and effectiveness will occur. <br /> 2.
