The medical waste management plan(MWMP) does not contain all information listed in HSC 117960. <br /> -MWMP shall contain all information listed in HSC 117960. HSC117960–Submit revised MWMP by 12/14/2015. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)–During this inspection facility was able to obtain treatment and tracking records from the <br /> month of March 2015 to this date. Facility spoke with a Stericycle representative and now understands how to <br /> obtain treatment records. No further corrective action needed. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant,tightly lid, clean, in good repair and <br /> appropriately labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, <br /> and be kept clean and in good repair. Container shall be labeled with the words`Biohazardous Waste"or <br /> "BIOHAZARD"on the lid and on the sides so as to be visible from any lateral direction. HSC 118280(c)–Add <br /> proper labeling to biohazardous waste containers that are missing labels in the patient area. Submit evidence <br /> f )mpliance by 12/14/2015. <br /> 26. Sharps waste is not placed in sharp container. Sharps container is not handled and/or labeled appropriately. <br /> - Sharps waste shall be placed in a sharps container. Full sharps container ready for disposal shall be taped closed or <br /> tightly lid and stored onsite not more than 30 days. Sharps container shall be labeled with the words"Sharps Waste" <br /> or BIOHAZARD". HSC 118285–Either add additional labels to sharps containers or ensure the containers are <br /> placed so that labels are visible. Submit evidence of compliance by 12/14/2015. <br /> 30. The designated accumulation area is not secured or marked with proper signage. <br /> -A designated accumulation area shall be secured by use of locks on entry doors,gates,or receptacle lids and marked with <br /> warning sign,"CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS KEEP OUT", in <br /> English and in another language determined to be appropriate by the infectious control staff or LEA.HSC118310–Ensure <br /> room is kept locked at all times. Either label the door or provide training to staff and submit evidence of compliance by <br /> 12/14/2015 <br /> Notes: <br /> 1. Facility contracts with Stericycle for the removal of medical waste. Waste is picked up every Monday, <br /> Wednesday, and Friday. <br /> 2. Facility stores medical waste in biohazardous waste bags compliant with ASTM D1709.
