HSC 118275(a)(1) Nitrile gloves were <br /> discarded in the lobby trash instead of a labeled,secured biohazardous waste container. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler, and medical waste is picked-up two or three times per week. <br /> 2) Pharmaceutical waste is picked-up at least four times per month in 2018 and at least three times per month in 2017. <br /> 3) The biohazardous waste bags in use meet both the ASTM 131922 and D1709 standards. <br /> 4) The Medical Waste Management Plan was available onsite with annual training records for the 25 employees that <br /> contact medical waste. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
