The medical waste management plan(MWMP) does not contain all information listed in HSC 117960. <br /> - MWMP shall contain all information listed in HSC 117960. HSC117960–Submit revised MWMP by 12/3/2015. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)– Obtain records showing treatment of waste from hauler. Submit evidence of compliance by <br /> 12/3/2015. <br /> 30. The designated accumulation area is not secured or marked with proper signage. <br /> -A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and marked <br /> with warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS <br /> KEEP OUT", in English and in another language determined to be appropriate by the infectious control staff or LEA. <br /> HSC118310–Add proper signage to both freezers. Both freezers must be kept locked unless the medical waste <br /> container(s) inside of the freezer is secured with a lock. Medical waste must be stored separate from other <br /> items inside of the freezer and the area where the medical waste is stored within the freezer must be labeled. <br /> Freezer located in Biology must retain partitions to separate medical waste from other materials in the freezer. <br /> Submit evidence of compliance by 12/3/2015. <br /> Reinspecion on/about: A reinspection fee of$130 per hour may be charged.
