Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD"on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c) <br /> 1. Biology Department: The biohazard waste container in the freezer has been replaced with a larger container that is <br /> missing labels on the sides. Add labels to the sides of the container and submit photographic evidence of compliance <br /> to the EHD by 2/25/2020. <br /> 2. Chemistry Department: Dr.Carlson's research laboratory in the department generates biohazardous waste. The <br /> waste is currently accumulated in an unapproved container. Biohazard waste is picked-up from the laboratory every <br /> three days and transported to the cold room in the Pharmacy Department for storage until collection by Ingenium. <br /> Provide a properly labeled biohazard waste container with liner for use in Dr.Carlson's laboratory,and submit <br /> photographic evidence of compliance to the EHD by 2/25/2020. (This violation was corrected during the inspection.) <br /> Photographic evidence of compliance is due to the department by 2/25/2020. <br /> Evidence can be submitted to the department at: jeaster(@sigov.orr <br /> Notes: <br /> 1) Ingenium is the licensed medical waste hauler for the facility. All Biohazardous waste is stored below 32 degrees F. <br /> Biohazardous waste is placed in bags and stored in a freezer in the Biology department and a cold room in the <br /> Pharmacy department. The waste is picked-up by Ingenium every 28 days. <br /> 2) The waste generated consists of sharps, bacterial cultures, potentially viruses, and animals used for dissection. <br /> 3) The Medical Waste Management Plan meets the requirements and training records were available for review. <br /> 4) Two type of biohazard waste bags are in use at the university. The bags meet both the ASTM D1709 and D1922 <br /> standards based on the manufacturer's specification.
