Pharmaceutical waste shall be segregated and contained pursuant to this section. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair <br /> and appropriately labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tightfitting <br /> cover, and be kept clean and in good repair. (This <br /> violation was corrected during the inspection) <br /> The facility uses Barnett Medical Service's website to access tracking documents for medical waste pick-ups. <br /> Reviewing the records online is arduous,time consuming,and requires more time than is covered by the current <br /> permit fee. If records are maintained electronically,they must be reviewable within the time covered by the current <br /> permit fee. If additional time is required,the facility will either be charged at the EHD's hourly rate of$152 an hour for <br /> additional time spent reviewing records or the permit fee for the facility will be increased to cover the actual time spent <br /> inspecting the facility. <br /> Notes: <br /> 1) Barnett Medical Services is the licensed medical waste hauler for the facility. Waste is picked-up two times per week. <br /> 2) Tracking records are available for the last two years. Destruction records are only available from August 2016 to July <br /> 2018. <br /> 3) Pharmaceutical waste is picked-up once or twice per month. Destruction records for pharmaceutical waste do not <br /> specify that destruction occurred by incineration. <br /> 4) The biohazardous waste bags meet both ASTM D1922 and D1709 standards. <br /> 5) Training records for training completed within the last year were available for employees. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
