This registered large medical waste generator does not maintain individual treatment records and shipping and <br /> tracking documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, <br /> and shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a) <br /> No shipping and tracking records were available for the month of October 2019. Submit evidence of shipments of <br /> medical waste off-site for the month of October 2019 to the EHD by 3/13/2020. <br /> 24. Pharmaceutical waste shall be segregated and contained pursuant to this section. The container shall be properly <br /> labeled. -Nonradioactive and non-RCRA pharmaceutical wastes,that are regulated as medical waste, shall be placed in <br /> a container or secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY" on the lid and sides, <br /> so as to be visible from any lateral direction. HSC 118275(a)(6)(B) <br /> The pharmaceutical waste container did not have a label on the lid. Submit photographic evidence of correction <br /> to the EHD by 3/13/2020. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tightfitting cover, and be <br /> kept dean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD" on the lid <br /> and on the sides so as to be visible from any lateral direction. HSC 118280(c) <br /> Two biohazard waste containers on the floor were lined and used for collection without lids. Material that has <br /> been in contact with blood,tubing containing blood, and other potentially infectious materials were observed in <br /> the containers without lids. Biohazard waste containers that contain waste must be lined and kept covered. <br /> Submit evidence of an in-service training on the collection and handling of biohazardous waste to the EHD <br /> by3/13/2020. <br /> All corrections are due to the department by 3/13/2020. <br /> Evidence of corrections can be submitted to the department atjeaster(a�sigov.org <br /> Failure to submit corrections by 3/13/2020 will result in a re-inspection charged at the EHD's hourly rate of$152 per hour. <br /> Notes: <br /> 1) The facility uses Barnett Medical Service's website to access tracking documents for medical waste pick- <br /> ups. Reviewing the records online is arduous,time consuming,and requires more time than is covered <br /> by the current permit fee. If records are maintained electronically,they must be reviewable within the time <br /> covered by the current permit fee. If additional time is required,the facility will either be charged at the <br /> EHD's hourly rate of$152 an hour for additional time spent reviewing records or the permit fee for the <br /> facility will be increased to cover the actual time spent inspecting the facility. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
