Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair <br /> and appropriately labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tightfitting <br /> cover, and be kept clean and in good repair. Container shall be labeled with the words <br /> "Biohazardous Waste"or"BIOHAZARD"on the lid and on the sides so as to be visible from any lateral <br /> direction. HSC 118280(c) The biohazardous waste container in the isolation room is missing a sticker on one side. (This <br /> violation was corrected during the inspection) <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler. Biohazardous waste is picked-up twice per week. <br /> 2) Pharmaceutical waste is picked-up at least once every 90 days. <br /> 3) The biohazardous waste bags meet both the ASTM D1922 and 131709 standards. <br /> 4) Annual training records were available for all employees handling waste. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
