Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD"on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c) <br /> The biohazard waste containers for the newly added Patient Dialysis area in Exam Room#1, Exam Room#2, and in the <br /> hall outside of the conference room were missing required labels(This violation was corrected during the inspection). <br /> Notes: <br /> 1) Shipping and tracking records were available for review. Stericycle is the licensed medical waste hauler for biohazard <br /> and pharmaceutical waste. Medical Waste is picked-up twice per week(on Wednesdays and Fridays). <br /> 2) The Medical Waste Management Plan was available for review. A copy was obtained for the department file. <br /> 3) Annual training is done online. Training records were available for the nine employees handling waste. <br /> 4) The biohazardous waste bags meet both the ASTM D1922 and D1709 standards. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
