Biohazard bag does not meet the standard set forth in HSC 117630. <br />- Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 <br />The facilities are using several different types of biohazard waste bags. Only one of the small bags meets both the <br />ASTM D1709 and the ASTM D1922 standards. Only bags that meet both the ASTM D1709 and the ASTM b1922 <br />standards can be used to ship biohazardous waste off site. Submit evidence of correction to the EHD by 3/19/2019. <br />24. Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained <br />pursuant to this section. The container shall be properly labeled. HSC 118275 <br />The pharmaceutical waste container was not labeled with the date of first use. Submit evidence of correction to the <br />EHD by 3/19/2019. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) <br />The biohazardous waste containers in use in the facilities are not labeled on the lid and all lateral sides. Submit <br />evidence of correction to the EHD by 3/19/2019. <br />All corrections are due by 3/19/2019. <br />Notes: <br />1) Stericycle is the licensed medical waste hauler for the facility. Medical waste is picked -up once per week. <br />2) Shipping and tracking records were submitted to the department for review. <br />3) An updated copy of the list of offices in the building was provided during the inspection. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
