DEC 2 9 2011 <br /> ENVIRONMENT HEALTH <br /> PERMIT/SERVICES <br /> December 28, 2011 <br /> Dear Sir or Madam: <br /> For the over 30 years that we have been in business, we have obtained an annual Medical Waste <br /> Limited Quantity Haulers Exemption(LQHE) from Stanislaus County. Our storage location is at <br /> the Alexander Cohen Hospice House at 2201 Euclid Rd., Hughson, CA 95326. Our treatment <br /> facility is Stericycle, Inc. at 4135 W. Swift Ave., Fresno, CA 93722. <br /> Las t year we were advised by a new nurse that at Visiting Nurse Association, her previous <br /> employer, that she had to carry a permit for every county through which she drove while <br /> carrying dirty sharps generated during the provision of patient care. When I contacted each <br /> county through which our nurses drive while carrying one quart sharps containers or in which we <br /> provide patient care, I was told that each nurse does need to carry a LQHE for each county, <br /> whether the sharps were generated in that county or not. The permit was also intended to protect <br /> the nurse in the event he/she was pulled over by law enforcement and found to have medical <br /> waste in the vehicle. As a result, we obtained six additional LQHEs for the nurses to carry in <br /> their vehicle. <br /> However, I have recently been informed by the Chief of the Medical Waste Management <br /> Program at the Department of Health that it is not necessary to obtain more than the Stanislaus <br /> County LQHE. Therefore, Community Hospice, Inc. will discontinue renewing our Medical <br /> Waste Limited Quantity Haulers Exemption with your county. <br /> I requested a letter in writing from Alison Dabney, Chief of the State Medical Waste <br /> Management Program as to why we do not need to continue to renew LQHEs from counties <br /> other than Stanislaus County. Please see the attached letter.
