Sampling and Analysis Chain-of-Custody Record tThe sampling and analysis chain-of-custody record is initiated at the time of sampling and contains, but is not limited to, the well number, sample type, analytical request, date of sampling, and the name of the sampler. The record sheet is signed, time logged, and dated by the sampler when IF-- transferring the samples. Custody transfers are recorded for each individual sample; for example,if i_ samples are split and sent to more than one laboratory, a chain-of-custody record sheet will accompany each sample. The number of custodians in the chain of possession will be kept to a minimum. A copy of the sampling and analysis chain-of-custody record is returned to EMCON with the analytical results. Groundwater Sampling and Analysis Request Form A groundwater sampling and analysis request form (see Figure A-4) or purchase order, or both, accompanies the samples delivered to each laboratory. These forms serve as official communication to the laboratory of the particular analysis/analyses required for each sample. At minimum, the groundwater sampling and analysis request form includes the following information: r • Date submitted • Specific analytical parameters k_ • Well number • Sample source j ANALYTICAL METHODS Samples collected as part of the monitoring program are analyzed consistent with accepted analytical procedures.
