• The lifting straps or cleaveses must be attached to the secure lifting points marked on the machines. <br /> • Tag lines must be used to steady the fusion machine during the lift. Personnel must never touch the <br /> fusion machine during the lift. <br /> • When placing pipe onto the fusion machine, personnel shall keep hands clear of pipe-securing device <br /> to avoid their hands from becoming pinched. <br /> • The welding iron should be kept clear of all combustibles and its electrical cord out of wet areas. <br /> • Hands must remain clear when using the pipe shaving knife to cut and straighten the area of pipe to <br /> be welded. <br /> • When moving pieces of pipe to be fused, a spotter must be used to prevent the pipe from wiping and <br /> contacting vehicles,personnel, and other property. <br /> • Personnel should never stand on the fusion machine while pieces of pipe are being loaded or <br /> unloaded. <br /> • Personnel need to maintain a safe distance from the pipe when loading and unloading. <br /> • Air monitoring must be conducted for methane, oxygen, hydrogen sulfide, and lower explosive limit <br /> at a minimum before entering pipe trenches. The action levels specified in Table 4 must not be <br /> exceeded. <br /> 5.1.13 Drilling Operations <br /> All operations involving the use of powered drilling rigs will follow generally accepted drilling practices <br /> (see Appendix E-Drill Rig Operations and Maintenance Manual). One person will be assigned the <br /> responsibility of Lead Driller. Additional personnel will assist with equipment as needed. The Lead <br /> Driller will be responsible for operating the drilling rig and ensuring safety. <br /> • General rules associated with drilling rig operations will be as follows: <br /> • An Exclusion Zone will be established around the bore hole using barricade tape as a physical barrier. <br /> • While drilling, all non-essential personnel shall remain at a distance which is past the radius of the <br /> boom,whenever possible. Workers remaining around the drill rig will be kept to a minimum. <br /> • Smoking is not permitted within 50 feet of a well boring. Smoking on the landfill footprint is <br /> prohibited. <br /> • All vehicles and heavy equipment must be parked at least 50 feet from drill rig when lowering mast. <br /> • All operators and crew members will be familiar with the rig operations and will have received <br /> practical training and have reviewed the "Drill Rig Operations and Maintenance Manual". (See <br /> Appendix E.) <br /> • All personnel should be instructed in the use of the emergency kill switch/shut down on the drill rig. <br /> • Hard-hats steel toed boots, goggles or safety glasses with side shields, hearing protection, and gloves <br /> for hand protection are required. <br /> 5-7 <br />
