• Informing field personnel of the potential hazards associated with the site <br /> • Summoning emergency response personnel <br /> • Notifying the HSM of an emergency <br /> • Verifying that all emergency equipment is routinely inspected and functional <br /> • Correcting any work practices or conditions that may result in injury to personnel or <br /> exposure to hazardous substances <br /> • Informing the appropriate emergency response agencies of the provisions made herein <br /> • Evaluating the safety of site personnel in the event of an emergency and providing <br /> evacuation coordination if necessary <br /> Maintaining site facilities and assisting site personnel in accessing those facilities <br /> The SS(or designee)will direct all emergency response activities conducted or managed by CB&I. <br /> 8.2 List of Emergency Contacts and Notification <br /> The (enter city name or agency here) Fire Department shall be contacted prior to initiating site <br /> activities. They shall be advised and notified about upcoming site activities and potential <br /> emergencies. This shall be done to ascertain response capabilities and to obtain a response <br /> commitment. <br /> The SS will be notified immediately in the event of an emergency. The SS will immediately <br /> evaluate the incident and, if necessary, notify emergency response personnel. If not previously <br /> notified, the HSM will be advised of the situation. Telephone numbers for emergency contact <br /> personnel are listed in Table 6. The list will be maintained with current contacts and telephone <br /> numbers, and will be posted at all CB&I -controlled telephones. <br /> The information provided to the emergency contact should include the nature of the incident and <br /> the exact location. Specifically,the information should include the following: <br /> • Name and telephone number of the individual reporting the incident <br /> • Location and type of incident <br /> • Nature of the incident(i.e., fire, explosion, spill, or release) and substances involved(if any) <br /> • Number and nature of medical injuries <br /> • Potential for additional risks or dangers <br /> • Potential off-site risks or dangers <br /> 8-2 <br />
