This Permit to Operate remains valid through the pernut--expiration "date* listed above, subject to payment of annual permit fees,and compliance with permit conditions and all applicable local, state, and federal regulations. This permit is valid only at,-thelocation specified above, and becomes void upon any transfer of ownership or location.
