C7 <br />4.0 KEY PERSONNEL AND RESPONSIBILITIES <br />4.1 Site Safety Personnel <br />Name <br />Responsibilities <br />Lawrence 'Butch' Stefani Site Manager <br />Ruben Ramirez Site Safety Officer <br />Lawrence 'Butch' Stefani Health and Safety Director <br />4.2 Forward Personnel and Responsibilities <br />The responsibilities of the Forward personnel listed in <br />Section 4.1 are outlined below. <br />4.2.1 Forward Site Manager <br />The Forward Site Manager, Butch Stefani, has the ultimate <br />responsibility for the health and safety of Forward personnel <br />at the Site. As part of his duties, Mr. Stefani shall be <br />responsible for: <br />• keeping the Forward Health and Safety Director informed <br />of Site developments <br />• ensuring that on-site Forward personnel receive the <br />proper training, and are informed of potential hazards <br />anticipated at the Site and procedures and precautions to <br />be implemented on the job <br />• ensuring that contractors and subcontractors are informed <br />of the expected hazards and appropriate protective <br />measures at the Site. (Subcontractors should also be <br />given a copy of Forward's HSP for review.) <br />• ensuring that resources are available to provide a safe <br />and healthy work environment for Forward personnel. <br />4.2.2 Forward Health and Safety Director <br />The Forward Health and Safety Director, Butch Stefani, shall <br />be responsible for: <br />• monitoring the health and safety impacts of this project <br />for on-site Forward personnel <br />0 HS -5 <br />
