IDSOLID WASTE FACILITY PERMIT Facility/PermitNumber: <br />39 -AA -0015 <br />17. LEA Conditions: <br />A. The combined Forward Landfill and the Forward Resource Recovery Facility permitted tons per operating day shall not exceed 6,680 tons(peak). <br />B. Forward Landfill's contaminated soils acceptance proroggram shall comply with the terms and conditions given in the LEA's letters to Forward dated October 31, 1990; <br />June 5, 1992; and January 7, 1994. <br />C. This facility shall comply with all terms and conditions of this permit. <br />D. This facility shall comply with all applicable State Minimum Standards For solid waste handling and disposal. <br />E. This facility shall comply with all Federal, State, and Local requirements and enactments, including all mitigation measures given in any certified environmental <br />document filed pursuant to Public Resources Code, Section 21031.6. <br />F. Any additional information concerning the design and operation of this facility must be provided upon request of the Local Enforcement Agency. <br />G. This permit is subject to review by the LEA and may be suspended, revoked, or modified at any time for sufficient cause. <br />H. This facility must conform to the Report of Disposal Site Information dated October of 1993 and the revised sections provided 2/1999. <br />I. The LEA reserves the right to suspend or modify waste receiving operations when deemed necessary due to an emergency, an actual or potential hazard, or creation of <br />a public nuisance. <br />J. A^ change that would cause a design or operation of this facility not to conform to the terms and conditions of this permit is prohibited. Such a change maybe <br />&'e <br />ered a significant change requiring a permit revision. In no case shall the operator implement any change without submitting a written notice of the proposed <br />to the LEA at least 120 days in advance of that change. <br />K. Safety equipment shall be provided for all employees at the landfill. <br />L. This facility must comply with the conditions of approval for the land use permit UP -91-12. <br />M. The operator shall maintain a copy of this permit at the facility to be available at all times to facility personnel and enforcement agency personnel. <br />N. A change in the operator of this facility shall require a new permit. <br />O. The Grifolyn TX -1200 (fire Retardant) synthetic blanket, or equivalent as approved by the LEA, is the only synthetic blanket approved for use at this facility. <br />P. Six inches of soil cover material shall be applied at the end of each operating week to the active face where the Griffolyn TX4200 (fire retardant) synthetic blanket <br />was used. <br />Q. An adequate supply of water or soil shall be kept near the active face to aid in fire suppression. <br />R. Application and removal of the synthetic blanket shall be done so as to minimize damage (puncrums,tears, ect.) to the synthetic blanket. <br />S. The operator shall remove from use and discard the synthetic blanket when it no longer functions as an adequate cover material as outlined in CIWMB's " Procedural <br />Guidance For the Evaluation of Alternative Daily Cover". <br />T. The LEA may order a synthetic blanket removed from service and discarded based on deteriorated condition(s) which prevent it form functioning as adequate cover. <br />U. Griffolvn TX -1200 (fire retardant) svnthetic blanket may only be used to cover non -hazardous solid waste mixed with de -watered sewa?e or water treatment sludge at <br />a minimum solid to liquid ratio of 5:1 by weight. The ultimate moisture holding capacity of the non -hazardous solid waste shall not exceeded. <br />V. Griffolyn TX -1200 (fire retardant) synthetic blanket shall not be used to cover designated waste, asbestos, or any other hazardous waste approved by DTSC for Class <br />Il or Class III disposal. Six inches of soil cover material still be used to cover these waste daily. <br />
