the TSM/SWL has reviewed and approved the material. The scale house attendant also records the date of delivery and quantity of ACW. The scale house attendant retains Forward's copy of the Hazardous Waste Manifest and gives the driver a Load Check Form that the driver must give to the Load Checker at the designated ACW disposal area before unloading. The scale house attendant also provides the driver with directions to the working face and the nearby designated ACW disposal area. Finally, the scale house attendant signs Hazardous Waste Manifest, documenting that Forward has received and disposed of the waste. Training: The scale house attendant receives in-house training with periodic updates on reviewing and processing manifests, notifications, and certifications required for proper ACW management, disposal, and record keeping. This includes training on internal controls such as the internal approval number and internal policies and procedures related to proper record keeping.
