XII. On October 3, 2003, this LEA prepared and sent to the operator the Notice and <br /> Order 03-01 for the violation of Explosive Gases Control, Section 20919.5 of <br /> 27CCR. <br /> XIII. On October 24, 2003, this LEA received the progress report for Notice and Order <br /> 03-01. According to the progress report, the landfill gas system expansion project <br /> was completed on October 2. 2003, and the vacuum had been applied to the new <br /> extraction wells. <br /> XIV. On October 24, 2003, this LEA conducted routine inspection and noted that the <br /> concentration of methane gas in the gas monitoring probe GP-9 (which replaces <br /> GP-2), located near the west boundary,was 40%. This was documented under the <br /> violation of Explosive Gases Control, Section 20919.5 of 27CCR. <br /> XV. On November 20, 2003, this LEA conducted routine inspection and noted that the <br /> concentration of methane gas in the gas monitoring probes GP-8 (which replaces <br /> GP-3) and GP-9 (which replaces GP-2),both of which were located near the west <br /> boundary, was 9% and 41%. respectively. This was documented under the <br /> violation of Explosive Gases Control, Section 20919.5 of 27CCR. <br /> XVI. On December 23, 2003,'this LEA conducted routine inspection and noted that the <br /> concentration of methane gas in the gas monitoring probes GP-8 (which replaces <br /> GP-3) and GP-9 (which replaces GP-2),both of which were located near the west <br /> boundary, was 9.5% and 44°o. respectively. This was documented under the <br /> violation of Explosive Gases Control, Section 20919.5 of.27CCR. <br /> XVII. On January 14, 2004, at the request of CIWMB, LEA amended the Notice and <br /> Order 03-01 so that the violations cited for the Explosive Gas Control were <br /> consistent for 3 consecutive months. <br /> XVIII. On January 30, 2004,this LEA conducted routine inspection and noted that the <br /> concentration of methane gas in the gas monitoring probes GP-8 (which replaces <br /> GP-3) and GP-9 (which replaces GP-2),both of which were located near the west <br /> boundary,was 48% and 450,,'0, respectively. This was documented under the <br /> violation of Explosive Gases Control, Section 20919.5 of 27CCR. <br /> VIOLATIONS: <br /> Consequently, the Forward Landfill has operated in violation of the following section(s) <br /> of 27CCR: <br /> 27CCR 20919.5 Explosive Gases Control <br /> (a) Owners or operators of all `iSWLF units must ensure that: <br /> (2) The concentration of methane gas does not exceed the lower explosive <br /> limit for methane at the facility property boundary. <br /> Amended Notice and Order 03-01 Page 3 of 6 Forward Landfill <br /> February 2,2004 <br />
